Medical charts almost always use side tabs in a 1/8 or 1/10 cut — more positions per bank than the office-standard 1/5, so a full chart's sections fit in one clean cascade down the page. A 1/8-cut tab is 1.25 inches tall; a 1/10-cut tab is 1.00 inch. Every tab extends 0.5 inches past the 8.5 × 11 sheet, so dividers finish at 9 × 11 and the tabs stay visible past the chart's contents.
