The file structure follows the course of care: how the patient arrived, what was found, what the plan is, and what happened at each visit. Most practices index around intake and history, the initial examination, imaging, the care plan, daily SOAP notes, re-examinations, and insurance and billing.

Daily notes are the section that grows fastest, which is why they usually get their own tab and often their own colour. A chart where the notes are separated from the plan and the re-exams is one that answers a payer's question in seconds rather than minutes.

Most charts are opened occasionally. A chiropractic chart is opened at every visit, often several times a week for the same patient across a course of care, and handled by front desk staff as much as by the doctor.

That volume of handling is the case for reinforcement. Poly is tear-resistant and wipes clean; Mylar-reinforced paper on 90# or 110# index stock is the economical choice and still resists tearing at the tab extension, which is where a heavily used divider fails first.

In a practice where the chart moves between the front desk, the adjusting room and billing, colour does more work than text. One colour per section means anyone can get to the right place without reading the tabs, which matters when the person pulling the file is not the person who wrote in it.

Paper chart sets run $8.00 per set at 25 sets, $3.58 at 100 and $2.44 at 250, setup included. Poly runs higher because the material costs more and lasts accordingly. Pricing is published and updates as you build the set — there is no quote step.
