Veterinary Chart Tabs

A human chart usually covers a condition or a course of treatment. A veterinary file covers a life: it opens at the first puppy or kitten visit and stays open through vaccinations, spay or neuter, dentistry, chronic disease management, and eventually end-of-life care. The sections have to hold years of accumulation without becoming a stack.

A small-animal clinic, an equine practice and an exotics service do not keep the same record. Equine files carry lameness exams, farrier and dental floating records, and Coggins testing. Exotics files vary so widely by taxon that the husbandry history often needs its own section. Mixed practices end up running two or three layouts.
