Agencies typically build around intake and consent, assessment, the treatment plan and its reviews, progress notes, medication and psychiatric records, coordination of care, and discharge.

Progress notes are the section that grows without limit, so most agencies give it its own tab and often its own colour. A chart where notes are separated from the plan is one a supervisor can audit quickly.

Behavioral health records are reviewed by people who did not write them — clinical supervisors, utilization review, payers, and accreditation surveyors. Each of those readers arrives looking for one section and judges the agency partly on how fast they find it.
