Resident Chart Tabs for Long-Term Care & Skilled Nursing
Tabzoola is a US manufacturer and supplier of custom resident chart dividers for long-term care, skilled nursing and assisted living facilities, made to order in Schaumburg, Illinois by Anselmo Die & Index, Inc. — die-cutting index tabs since 1993 and shipping nationwide.
Long-term care is one of the settings where paper charts genuinely persist. Electronic records handle assessments and billing, but the resident chart at the nurses' station still holds signed documents, orders, flow records and the paperwork that has to be handed to a surveyor or a family member without anyone logging in. Those charts get opened dozens of times a day, by staff on every shift.
That combination — high handling, many identical files, and documents someone external may ask to see — is what makes tab structure worth deciding once and printing. Tabzoola prints resident chart dividers to your facility's own section list, collated so every chart in the building is assembled the same way.
The sections facilities commonly tab
Resident charts are wider than a clinic chart because they carry the care-planning and daily-record side as well as the clinical one. A representative structure, renamed to match your own forms:
- Face Sheet / Admission — admission record, demographics, insurance, responsible party
- Physician Orders — standing and telephone orders, order recaps
- Care Plan — the plan of care and its revisions
- MDS / Assessments — assessment documentation and supporting worksheets
- Progress & Nursing Notes — interdisciplinary and shift notes
- Medications / MAR — medication administration records, allergies, pharmacy communications
- Labs & Diagnostics — results, imaging reports, consult results
- Therapy — PT, OT and speech notes and certifications
- ADL / Flow Records — daily care and activities-of-daily-living documentation
- Advance Directives & Consents — signed directives, consents, authorizations
- Social Services & Activities — assessments, notes, care conference records
- Dietary / Nutrition — diet orders, nutrition assessments, weights
Why consistency matters more here than almost anywhere
In a facility, the person pulling a chart is rarely the person who assembled it. Night shift, agency staff, therapy, a consultant pharmacist and a surveyor all open the same binder expecting the same order. When two units file the same document in different sections, retrieval turns into a search.
Printed, collated sets remove that variance by construction: identical wording, identical order, in every chart on every unit. Empty tab positions also do quiet work — a section with nothing behind it is visible immediately, rather than discovered by someone looking for a document that was never filed.
Built for the way facilities actually buy
Resident charts are consumed steadily rather than bought once: every admission needs a new chart, and long stays outgrow the first binder and get split into volumes. That makes the volume forecastable, which is why facilities tend to order the year in a run rather than reordering monthly.
Multi-facility groups get the additional benefit of one structure across every building — the design is saved, so a second location orders an identical set without re-specifying it, and a corporate consultant reviewing charts sees the same binder everywhere.
- Minimum 25 sets; volume pricing past 1,000 for facility or group orders
- The saved design means every reorder matches the original exactly
- Chart thinning: both volumes get the same section structure, so nothing depends on which volume someone pulls
Materials for charts handled every shift
Resident charts are among the highest-handling binders in healthcare. Solid .015 poly wipes clean, resists moisture, and is effectively tear-proof — the usual pick for the active chart at the station. Mylar-reinforced 90# or 110# paper costs less and still protects the tab extension, which is where unreinforced dividers fail and where a torn tab turns a section into loose paper.
Both are letter size, 3-hole punched for standard chart rings, and print up to two lines per tab, one or both sides. Color-code one section per color and staff read the section without reading the tab.
Ordering, and what we do not do
Minimum 25 sets, priced live online with no quote request. Paper produces in about 5 business days from proof approval, poly in about 15. Purchase orders are accepted by email with Net-30 invoicing, and a free PDF proof routes to whoever signs off before anything prints.
Tabzoola manufactures the chart dividers. It does not provide clinical, documentation or regulatory guidance, and nothing here should be read as advice on what a resident record must contain, how long it must be retained, or what a surveyor will require. The section list belongs to your facility's policies, your DON or administrator, and your own consultants; we print it exactly as given.

Frequently asked questions
What are resident chart tabs?
Printed divider tabs that split a long-term care resident chart into labeled sections — face sheet, physician orders, care plan, assessments, progress notes, medications, labs, therapy, ADL records, advance directives and more. Custom sets print the facility's own section wording and arrive collated so every chart is assembled identically.
How many sections does a resident chart need?
Most facilities run 10 to 12 sections, which is wider than a clinic chart because the care-planning and daily-record side needs its own tabs. Ten to twelve sections suits the 1/10 and 1/12 cuts; every cut from 1/2 to 1/15 is available. Past about fifteen tabs in one bank the labels get too narrow to read, so extra sections step into a second staggered bank.
Poly or paper for resident charts?
Poly for the active chart at the nurses' station — solid .015 plastic that wipes clean, tolerates gloves, and does not tear. Mylar-reinforced paper for closed records and cost-sensitive volume; the Mylar edge still protects the tab extension where unreinforced dividers fail. Many facilities run poly active and paper for stored volumes.
Can every building in our group use the same set?
Yes, and that is the usual reason groups order custom. The design is saved after the first order, so any facility reorders an identical set without re-specifying it — same wording, same colors, same order — which is what lets a corporate reviewer open any chart in any building and find the same structure.
What is the minimum order for a single facility?
Twenty-five sets, priced live online with setup included and a free PDF proof before production. That suits a single building stocking ahead for admissions; volume pricing runs past 1,000 sets for facility-wide or multi-site orders.
Do you advise on what sections our charts should have?
No. Tabzoola manufactures the dividers and prints the section list you provide. What a resident record must contain, how it is retained, and what a survey requires are governed by your facility's policies and applicable regulations — decisions for your administrator, DON, or compliance consultant, not a tab manufacturer.
