Tabzoola is a US manufacturer and supplier of custom hospital chart dividers for inpatient nursing units, made to order in Schaumburg, Illinois by Anselmo Die & Index, Inc., which has die-cut index tabs since 1993. A 12-tab inpatient chart set costs $11.66 a set at the 25-set minimum and $3.86 at 250, collated and 3-hole punched.

Hospitals that run electronic records still keep a paper chart at the unit for the documents that need a signature or have to be on hand when systems are down: orders, consents, flow sheets and the downtime packet. Tabzoola prints those chart dividers to the hospital's own section list, so a float nurse, a resident and a unit clerk all find Physician Orders in the same place on every chart.

Tabzoola makes hospital chart dividers two ways: Mylar-reinforced paper for charts that are thinned and archived, or solid .015 poly that wipes clean for charts handled with gloves at the bedside and the nurses' station.

The sections an inpatient chart usually carries

Inpatient charts run longer than clinic charts because nursing documentation sits beside the physician record. A representative 12-section structure, renamed to match your forms:

  • Face Sheet: admission record, demographics, contacts and insurance.
  • Physician Orders: admission, standing, telephone and verbal orders.
  • History & Physical: the admitting H&P and updates.
  • Progress Notes: physician and interdisciplinary notes.
  • Nurses Notes: shift assessments and nursing narrative.
  • MAR: medication administration records.
  • Flow Sheets: vitals, intake and output, and graphic records.
  • Labs: results and pathology.
  • Radiology: imaging reports.
  • Consults: specialist consults and referrals.
  • Consents: signed consents, advance directives and releases.
  • Discharge Planning: case management, education and discharge instructions.

One set for every unit, or one per service line

Most hospitals pick one of two approaches. A single house-wide set means any chart on any floor opens the same way, which helps float staff and the health information management team that thins and scans charts. Separate sets by service line suit units whose documents differ: labor and delivery adds Prenatal Records and Delivery Record, surgical units add Anesthesia and Operative Report, behavioral health adds Treatment Plan and Legal Status.

Tabzoola saves each design, so every unit reorders exactly the same set, and a new section can be added for one unit without changing the others. The price follows tabs per set and number of sets, not the wording, so a unit's own set costs the same as the house set.

Cut, size and material for unit charts

Twelve sections fit in one bank at a 1/12 cut, which keeps every tab visible in a single cascade down the chart. Units with fewer sections use the 1/8 or 1/10 cut for wider, easier-to-read tabs. Sheets are letter size with a half-inch tab, so dividers finish at 9 x 11 and stay visible past the chart contents in standard chart binders and ring holders.

Solid .015 poly is the choice for the active chart at the bedside: one piece of plastic, wipeable with standard surface disinfectants and effectively tear-proof. Mylar-reinforced 90# or 110# paper costs less and suits charts that are thinned to storage. Both are printed with your section titles, collated and 3-hole punched; other ring patterns are quoted.

Ordering for a hospital

The minimum is 25 sets, priced instantly online with volume breaks to 1,000 sets and beyond on request. Every order gets a free PDF proof before anything prints. Paper ships about 5 business days after proof approval and poly about 15. Purchase orders are accepted with Net 30, and one order can be split across units or shipped to more than one building.

Tabzoola manufactures the dividers. The section list comes from the hospital's own policies and forms; Tabzoola does not give clinical, documentation or records-retention guidance.

Frequently asked questions

Who makes custom chart dividers for a hospital unit?

Tabzoola, a US manufacturer in Schaumburg, Illinois, makes custom hospital chart dividers printed with a unit's own section names, such as Physician Orders, Nurses Notes, MAR and Flow Sheets. Sets are collated and 3-hole punched, in Mylar-reinforced paper or wipe-clean poly, from a 25-set minimum with a free PDF proof.

How much do hospital chart dividers cost?

At Tabzoola a 12-tab inpatient chart set is $11.66 a set at the 25-set minimum, $5.48 at 100 and $3.86 at 250, collated and 3-hole punched, setup included. Your own section names cost the same as the standard list.

What sections go in a hospital inpatient chart?

A common 12-section inpatient chart is Face Sheet, Physician Orders, History & Physical, Progress Notes, Nurses Notes, MAR, Flow Sheets, Labs, Radiology, Consults, Consents and Discharge Planning. Hospitals rename and reorder these to match their own forms and policies.

Can each hospital unit have its own chart divider set?

Yes. Tabzoola saves each design, so a hospital can run one house-wide set or separate sets for units such as labor and delivery, surgery or behavioral health. Each set reorders exactly as approved, and every set has the same 25-set minimum.

Paper or poly chart dividers for a hospital?

Choose solid .015 poly for active charts at the bedside: it wipes clean and resists tearing. Choose Mylar-reinforced paper for charts that are thinned and stored, where it costs far less per set. Both are printed, collated and 3-hole punched.

Does Tabzoola take hospital purchase orders?

Yes. Tabzoola accepts purchase orders with Net 30 terms, and one order can be split across units or shipped to several buildings. Every order includes a free PDF proof before anything prints.

Amazing Customer Service

We had a deadline to get 50 presentation books out, and our current tab supplier couldn't deliver the tabs that we needed on time. I worked directly with Justin and he was able to get our order manufactured and shipped out within days. The order arrived in time for us to complete our presentations, and the tabs looked great. This type of amazing customer service is very hard to find nowadays, but I'm thrilled to now have a supplier that I can count on.

Lamar W. · Sep 1, 2026
Response from Tabzoola

Lamar, thank you — a deadline like yours is exactly what the shop is built for. The moment Justin heard it, your deadline became our deadline, and your order was manufactured and out the door in days. Rush work only counts when it arrives looking right, so "the tabs looked great" is the part we're proudest of. Count on us for the next one — we'll be here. John Anselmo, Tabzoola

Megan, thank you for the kind words — I'll pass them straight to Justin. Getting every question answered before anything prints is exactly how we like to work, so "now I feel like an expert" is about the best review we could ask for. Whenever your next set of tabs comes up, your questions are always welcome. John Anselmo, Tabzoola

Great customer service

I can't say enough about how great Justin was helping me with my order. After I approved the order he took time to call me to ask if i was sure about a decision to use next day delivery on the whole order when only a partial would keep me going while the rest of the order shipped ups ground. He saved me a ton of money. Thank you for caring about your customers. I wish more people were like you.

Amy L. · Aug 20, 2026
Response from Tabzoola

Thank you, Amy — I'll make sure Justin sees this. Splitting the shipment so only the part you needed went next-day was the right call, and it's the kind of thing we'd rather catch on the phone than have you find on the invoice. Anyone here would have made that call. Thank you for taking the time to write it down. — Tabzoola

Response from Tabzoola

Thank you, Perri — losing a supplier you've used for years is a real headache, and matching a set you already rely on is the kind of job we like. If you ever need another one matched, a photo of an existing tab is enough for us to pick up the titles and the colors. Glad we could keep you running. — Tabzoola

Perfect!

Tabs came out better than expected. Will definitely be back.

Joan S. · Aug 18, 2026
Response from Tabzoola

Thank you, Joan — better than expected is a good place to land. The part people usually don't expect is the make: real die-cut tabs on index stock, with the Mylar bonded on both sides of the extension, which is where tabs tear first. We'll be ready whenever you need the next run. — Tabzoola
