Chart divider questions

What sections go in a medical chart?

There is no single required standard — it depends on your specialty and how your practice charts. Common sections for a general/family practice are Progress Notes, Labs, Imaging, Medications, Immunizations, Consults, Correspondence, and Billing. Cardiology adds EKG and Echo; inpatient charts add a Face Sheet, Physician Orders, and a Medication Administration Record (MAR). This tool gives a sensible starting set for several specialties that you can rename and reorder to fit your office.

How should I color-code chart sections?

Assign one color per section and use it consistently on every chart, keeping neighboring tabs high-contrast so staff can find a section from across the room without reading labels. A common scheme is Progress Notes blue, Labs yellow, EKG/diagnostics red, Consults green, Medications orange, and Misc gray. On custom tabs the color lives in the Mylar tab (19 colors) or the whole poly divider (11 colors).

How do I turn this list into printed chart dividers?

Build your section list here, click Copy, then open the Tabzoola designer and choose “Paste a list.” Each section lands on its own tab in order. Pick each tab's color to match your scheme, choose wipe-clean poly or Mylar-reinforced paper, add 3-hole punching, and approve a free PDF proof before anything prints. Reorder the identical set anytime as your practice grows.
