1.  PRODUCT. Custom printed index tab dividers: die-cut divider sheets with
    printed tab extensions, manufactured to order.

2.  SHEET SIZE. ____ (standard: 8.5" x 11" letter; finished size including a
    0.5" tab extension is 9" x 11").

3.  TAB CUT. 1/____ cut (____ tab positions per bank). Vendor shall confirm
    availability of all cuts from 1/2 through 1/15.

4.  TABS PER SET. ____ tabs per set. Sets exceeding one bank shall run as
    staggered banks; vendor to propose the bank arrangement. Vendor shall state
    the maximum tabs per set supported (sets up to 150 tabs may be required).

    (a spread such as 100 / 250 / 500 is suggested) and shall state whether
    typesetting, die, and setup charges are included in the per-set price or
    billed separately.

13. PROOF. Vendor shall provide a PDF proof showing each tab exactly as it
    will print, at no charge, and shall not begin production until the proof
    is approved in writing. Vendors able to provide the proof at or before the
    time of quotation are preferred.

14. TURNAROUND. Vendor shall state production time in business days from
    written proof approval.

15. MINIMUM ORDER. Vendor shall state the minimum order quantity. Minimums
    greater than 25 sets shall be identified in the bid.

16. ORIGIN. Vendor shall identify the country of manufacture.
